PRIOR TO YOUR INITIAL VISIT, you must provide the following information:
•	a completed registration questionnaire and medical questionaire form (sent to you by our office)
•	a referral slip and recent radiographs from your referring dentist. Your dentist can send digital radiographs to ekelmanperio@gmail.com one day prior to your appointment. If radiographs are more than one year old or incomplete, we will take digital (computerized) radiographs. These x-rays will be electronically sent to your dentist along with a diagnostic report after your initial visit.
•	a completed insurance information. If your spouse or partner is the primary subscriber, be sure to have their information available for proper and expeditious insurance processing. Please click here for insurance information
•	Please notify the office if you have a medical condition or concern prior to a surgery (artifical heart valves, blood thinner medication, history of bacterial endocarditis, diabetes, organ transplants, etc). Please note new American Heart Association recent revision for antibiotic prophylaxis: www.ada.org Please inform Dr. Ekelman if you are currently taking intravenous (IV ) bisphosophonates or oral bisphosohphonates.
